When Is Wisdom Tooth Extraction Necessary?

In some instances, all four wisdom teeth erupt in perfect alignment, causing no issues to your bite or your adjacent teeth. Unfortunately, this is a rare exception. In most cases, wisdom teeth erupt malpositioned due to lack of adequate space in the jaw. Third molars that are only partially erupted can harbor bacteria, resulting in an increased risk for decay and gum disease. Sometimes wisdom teeth remain completely hidden, fully impacted in the jawbone. In these cases, cysts or tumors can develop around the wisdom tooth which can lead to problems with surrounding bone or neighboring teeth and necessitate future surgery.

What to Expect during Treatment

Knowing what to anticipate during your procedure can help you prepare for a successful and comfortable recovery. In the sections below, we will explore the various stages of treatment: imaging and treatment planning, wisdom tooth removal, and healing and recovery. 

Imaging and Treatment Planning

During your initial consultation, we will take digital x-rays and in certain cases a CBCT scan to assess the position of your wisdom teeth and to evaluate the surrounding anatomic structures. The images obtained during your first visit will help our doctors determine the appropriate course of treatment.

Wisdom Tooth Removal

Wisdom tooth removal can be performed under sedation, which induces a state of relaxation and increases your comfort level. In addition to sedation, your gums will be numbed with local anesthesia. To begin the procedure, the surgeon will make a small incision in the gums around the wisdom tooth.

Using special dental instruments, the tooth will be separated from the surrounding tissue. The tooth is then lifted from its socket. In some cases, the tooth is divided into segments for easier removal. Finally, the incision is closed with stitches, and a piece of gauze is placed over the extraction site. 

Healing and Recovery

Following wisdom tooth extraction, some level of discomfort is to be expected. However, this can be managed with prescription medications and over-the-counter pain relievers. In most cases, patients fully recover in three or four days. Depending on the complexity of your case, it could take up to one week. During the healing process, it is important to closely follow the guidelines set forth by your doctor. This typically means:

  • No strenuous exercise for at least 48 hours after surgery
  • No vigorous swishing or spitting
  • Do not drink through a straw for one week
  • Do not smoke during recovery, since it can slow the healing process
